One of the blog project groups (Self-Determination) is experimenting around with using a survey for a portion of their blog. They've just developed a quick & easy survey so they could learn the ins-and-outs of using the Google Docs form feature. Check it out:

They are using the classic Likert-type survey structure. But you don't have to use that particular structure at all. In the image below you can see all the different formats you can use for a Google form:

You can mix and match question types. And the very nice thing is Google Docs will automatically collate the data for you into one handy-dandy spreadsheet which you can subsequently export as an Excel file or as a regular text file (depending on your analysis needs).
